SF 3112 Checklist: Every Section of the Disability Retirement Form Explained
What the SF 3112 Actually Is
Standard Form 3112 isn't a single form — it's a five-part series that covers the disability-specific portions of a FERS (or CSRS) disability retirement application. Each part is completed by a different person, and the entire series must be submitted alongside SF 3107 (the general retirement application) and proof of your SSDI filing.
OPM publishes all five parts as a combined PDF, which can make it look like one long form. It's not. Each section has its own purpose, its own required signer, and its own set of traps that can delay or sink an application.
SF 3112A: Applicant's Statement of Disability
Who fills it out: You, the applicant.
What it asks: Your medical condition, when it began, how it affects your ability to perform specific duties, what accommodations you've requested, and whether those accommodations resolved the limitations.
How to approach it: This is your narrative, and it needs to be specific. Don't write a medical history — your doctor handles that on SF 3112C. Write a work history. Describe what you used to be able to do, what you can no longer do, and exactly how your condition makes those duties impossible.
Bad example: "I have chronic back pain that prevents me from working effectively."
Better approach: "My degenerative disc disease causes radiating pain when seated for more than 20 minutes. My position as a Claims Examiner requires reviewing case files at a workstation for 6–8 hours per shift. I can no longer maintain the sustained seated posture this requires, even with the ergonomic chair accommodation my agency provided in March 2025. My error rate on case reviews has increased from 2% to 14% since September 2025, as documented in my most recent performance review."
Reference your position description by name and duty. Mention specific accommodations attempted and why they failed. If your attendance has suffered, state the numbers — days of sick leave used, FMLA leave taken, LWOP periods.
SF 3112B: Supervisor's Statement
Who fills it out: Your immediate supervisor.
What it asks: Your performance and attendance record, documented deficiencies, accommodations the agency attempted, and the supervisor's assessment of whether you can continue performing your duties.
What you can do: You can't fill this out, but you can prepare for it. Before your supervisor sits down with the form, make sure your performance record includes documentation of the issues — leave records, performance counseling memos, accommodation request correspondence.
If your supervisor downplays the impact — writing "performance has been satisfactory" when you've been absent 40 days in six months — that inconsistency can undermine your application. A supervisor who accurately documents the pattern of decline is helping your case, even though the documented deficiencies feel uncomfortable to read.
If your supervisor has been transferred or retired, your current supervisor or the next person in the chain of command fills out SF 3112B based on available records.

SF 3112C: Physician's Statement
Who fills it out: Your treating physician.
What it asks: Clinical diagnosis, treatment history, prognosis, and a detailed assessment of how the medical condition limits your ability to perform your job duties.
Why it matters most: SF 3112C is the single most important document in the package. OPM's Disability Division lives and dies by the physician's functional assessment. A vague statement — "patient is disabled and cannot work" — gets denied. A detailed statement linking clinical findings to position description duties gets approved.

SF 3112D: Agency Certification of Reassignment and Accommodation Efforts
Who fills it out: Your agency's HR office or Reasonable Accommodation Coordinator.
What it asks: What accommodations were attempted, whether they were effective, whether a reassignment search was conducted, and the results of that search.
What to watch for: This form must certify that the agency searched for vacant positions at the same grade, pay level, and commuting area — and either found nothing or offered something you accepted/declined. If HR hasn't actually conducted the reassignment search, they can't certify this form, and your package is incomplete.
Push HR to complete the search promptly. Some agencies have a formal 30-day search window; others drag the process out for months. Follow up in writing so you have a record of your diligence if the one-year filing deadline becomes an issue.
SF 3112E: Disability Retirement Application Checklist
Who fills it out: The agency's HR specialist.
What it is: A transmittal checklist confirming that all required pieces of the package are assembled before submission to OPM. It includes checkboxes for each form, supporting medical documentation, proof of SSDI filing, and any supplementary evidence.
This form is straightforward, but it serves an important quality-control function. If your HR specialist completes SF 3112E and checks every box, the package should arrive at OPM without missing pieces that trigger a request for additional information and add months to processing.
Assembly Tips
Submit everything together. OPM processes packages faster when they arrive complete. Missing forms or documents trigger information requests that can add 30–60 days to the timeline.
Keep copies of everything. Once your package goes to OPM, you may not see those forms again unless you request your file. Scan every completed form, every piece of medical documentation, and your SSDI proof of filing before submission.
Follow OPM's digital submission path. Since July 2026, agencies submit through the Online Retirement Application (ORA) portal. Digitized claims move faster through OPM's system than paper files.
Don't wait for every piece to be perfect. If you're approaching the one-year filing deadline and one form is delayed, submit what you have as a protective filing and supply the remaining documents afterward.
