SF 3107 for Disability Retirement: What You Need to Know
The Role of SF 3107 in a Disability Application
Standard Form 3107 — Application for Immediate Retirement Under FERS — is the general retirement application that every FERS retiree fills out, whether they're retiring voluntarily or on disability. It's not a disability-specific form (that's the SF 3112 series), but it's a required part of your disability retirement package.
Think of SF 3107 as the administrative foundation. It captures who you are, your service history, who your beneficiaries are, and what survivor benefit elections you're making. The SF 3112 forms sit on top of this foundation and add the disability-specific medical and accommodation evidence.
Key Sections to Get Right
Personal Information and Service History
This section is straightforward — your name, date of birth, social security number, agency, position title, and grade. The critical piece is making sure your service history is accurate and complete.
If you've worked for multiple federal agencies, all service periods must be accounted for. Your agency's HR office prepares SF 3107-1 (Certified Summary of Federal Service) to document your complete civilian and military service record.
Military service: If you have military service you want credited toward your annuity computation, you must have completed a military service deposit. A deposit does not count military time toward the 18-month civilian service minimum for disability retirement eligibility.
Survivor Benefit Elections
This section requires careful thought, especially if you're married. Under FERS, you can elect a full survivor annuity (50% of your unreduced annuity payable to your surviving spouse), a partial survivor annuity, or no survivor annuity.
If you elect anything less than the full survivor annuity, your spouse must sign SF 3107-2 (Spouse's Consent to Survivor Election), and the signature must be notarized. This isn't optional — OPM won't process the application without it if you're reducing or waiving the survivor benefit.
The survivor election has a real cost: electing the full survivor annuity reduces your own monthly annuity by approximately 10%. For disability retirees, this reduction applies during the 60/40 formula period and carries through to the age 62 recalculation.
Consider this decision carefully. If your spouse has their own retirement income or is significantly younger and may remarry, the calculus is different than if they depend entirely on your federal benefits.
FEGLI Life Insurance
SF 3107 includes the section where you elect which FEGLI coverage to carry into retirement. You'll also complete SF 2818 (Continuation of Life Insurance Coverage) to formalize these choices.
The key decision points:
- Basic coverage costs relatively little and continues automatically if you've had it for the five years immediately before retirement
- Option A (Standard additional coverage) continues at a modest premium
- Option B (multiples of salary) gets expensive fast — premiums are based on five-year age bands and can double at ages 45, 50, 55, and 60
- Option C (family coverage) follows similar age-band pricing
For employees approaching age milestones, review the FEGLI premium schedule before electing to continue Option B multiples. The cost often exceeds the value of the coverage, and converting to a private policy within 31 days of retirement may be more economical.
Direct Deposit
Your annuity payments go to the bank account you designate on SF 3107. Double-check the routing and account numbers — errors here cause payment delays that compound the already-long processing timeline.
Filing SF 3107 for Disability Retirement
If you're still employed or within 31 days of separation: Submit SF 3107 to your agency's HR office along with the complete SF 3112 package. HR processes it through OPM's Online Retirement Application (ORA) portal.
If you've been separated more than 31 days: Submit SF 3107 directly to OPM's Retirement Operations Center in Boyers, Pennsylvania. Your former agency may no longer have your personnel records, so you'll need to coordinate with them separately for the agency-completed portions of the SF 3112 series.
Protective filing: If you're approaching the one-year post-separation deadline and your SF 3112 package isn't complete, submit SF 3107 along with SF 3112A directly to OPM. This preserves your filing deadline while you work on getting the remaining forms completed.

Common SF 3107 Mistakes in Disability Cases
Leaving the survivor benefit section blank. OPM will return the form. If you're married, you must make an explicit election. If you're electing less than the full survivor annuity, SF 3107-2 must be attached with a notarized spousal signature.
Inconsistent service history. If your SF 3107 shows different service dates than what HR certifies on SF 3107-1, OPM will request clarification — adding weeks to processing.
Counting military time toward the 18-month service minimum. The 18-month eligibility requirement is civilian service only. A military service deposit does not satisfy it. If you have fewer than 18 months of civilian FERS service, your application will be denied for insufficient service.
Not understanding the disability-specific annuity. SF 3107 was designed for voluntary retirement, so some of its language assumes the standard FERS formula. For disability retirement, the 60/40 formula applies during the first 12 months and from month 13 to age 62 — not the 1% per year computation shown in some SF 3107 worksheets.
