FERS Retirement Paperwork Checklist: Every Document You Need

The Problem With Incomplete Packages

A missing document doesn't just slow your FERS retirement application — it stops adjudication entirely until the document arrives. OPM sets your file aside, moves to the next case in the queue, and your application re-enters the line once the gap is filled. With the current average processing time at 98 days for complete digital applications, an incomplete package can push that well past six months.

The challenge is that the required documents come from multiple sources — your eOPF, military pay centers, courts, notaries, and your payroll office — each with their own lead times. Gathering everything after you've filed is too late. This checklist organizes the documents by category so you can work through them systematically before submission.

Category 1: Core Application Forms

SF 3107 (Application for Immediate Retirement under FERS): The main application. All six pages must be completed, signed in blue or black ink, and dated. Download the current version (revision 05/2014) from OPM's Standard Forms Directory — outdated versions circulating on third-party sites will be rejected.

SF 3107-1 (Certified Summary of Federal Service): Prepared by your agency HR office. It summarizes all periods of civilian and military service, retirement coverage codes, and your accumulated sick leave balance. Review it against your eOPF records, sign Section E, and return the original.

SF 3107-2 (Spouse's Consent to Survivor Election): Required only if you're married and electing less than the full 50% survivor annuity. Must be notarized — the spouse's signature and notary's signature must share the exact same date, with no corrections, white-out, or cross-outs anywhere on the form.

SF 2818 (Continuation of Life Insurance Coverage): Documents your FEGLI election for retirement. Select your post-65 reduction option (75%, 50%, or no reduction) and confirm the coverage multiples you're continuing.

Category 2: Identification and Marital Records

Marriage certificate: A clear, legible copy of your current marriage certificate. This establishes survivor annuity eligibility and connects to the elections you make in Section D of SF 3107.

Certified divorce decrees: If you have any prior marriages that ended in divorce, attach certified copies of every divorce decree, dissolution agreement, and court-approved property settlement. If a former spouse holds a court order apportioning retirement benefits (COAP), confirm that OPM's Court-Ordered Benefits Branch has a copy on file.

Foreign-language translations: If any marital documents are in a language other than English, include a certified English translation alongside the original.

Birth certificate or passport: Verifies your age eligibility and supports survivor annuity calculations.

Category 3: Military Service Records

DD-214 (Member Copy 4): Required for each period of active military service. This specific copy shows discharge character, rank, and exact service dates — other copies may omit critical fields.

RI 20-97 (Estimated Earnings During Military Service): Completed by the military pay center. Used to verify and calculate the military service deposit amount.

Military deposit paid-in-full receipt: The confirmation from your agency payroll office proving the post-1956 military service deposit was settled before separation. Without this, OPM will not credit military service toward your FERS annuity.

Civilian service deposit/redeposit receipts: If you had periods of temporary service or non-deduction civilian employment, attach receipts proving those deposits or redeposits have been paid.

Category 4: Insurance Continuity Documentation

FEHB/PSHB enrollment proof: Your agency should document that you've maintained continuous health plan enrollment for the five years immediately preceding retirement (or since your earliest opportunity to enroll). Pull your SF-2809 forms from your eOPF to verify this independently.

TRICARE or CHAMPUS proof of coverage: If you're using military health coverage to satisfy the five-year continuity requirement, attach an official proof-of-coverage letter demonstrating uninterrupted enrollment.

Category 5: Financial and Tax Forms

Direct deposit verification: Confirm that the routing and account numbers in Section F of SF 3107 are correct. Cross-reference against a voided check or your bank's official documentation — transposed digits in a routing number mean your interim payments go nowhere.

Form W-4P (Withholding Certificate for Periodic Pension Payments): Designates your federal income tax withholding preferences. If you skip this form, OPM defaults to single-filer status with zero adjustments, which typically overwitholds.

Category 6: Conditional Forms

SF 3108 (Service Credit Payment Application): Required if you're making a deposit or redeposit for civilian non-deduction service.

SF 3102 (Designation of Beneficiary): Names the beneficiaries for any lump-sum payment of remaining retirement contributions upon your death. This is separate from your survivor annuity election.

SF 2823 (Designation of Beneficiary for FEGLI): Names your life insurance beneficiaries. Without this form on file, FEGLI benefits follow the default order of precedence: spouse, then children, then parents, then estate.
